Partial Requirements
1. Duly filled out Borrower and Co-Borrower application forms. See attached file for reference.
2. (2) Two Government IDs of borrower and co-borrower
3. Business Registration· Sole Proprietorship DTI Registration Mayor's Permit· Corporation/Partnership Sec Registration Updated General Information Sheet (GIS)o Articles of Incorporation and By Laws Mayor's Permit
4. Latest (3) three months bank statements
5. Income Tax Return (ITR) and Audited Financial Statement (AFS) - (If Available)
6. Collateral - (If Available)· Real Estate Mortgage (REM)o Title and Tax Declaration· Chattel Mortgages Official Receipt (OR) and Certificate of Registration (CR)

Note: - Please ensure that all fields of the application form are completely filled out.- Keep in mind that these are partial requirements. Once your application passes the first evaluation, one of our officers will contact you to ask for additional documents to continue the process. Thank you.
